He was a brillliant surgeon who suddenly became paranoid of seeing blood. (A horrible surgery experience caused that reaction to him.)
So he leaves London to become a General Practice doctor out in the rural countryside---to allay his fears and 'get over' the blood thing eventually.
Well, of course he starts out on the wrong foot with practically everyone---since, while he's absolutely a genius doctor, he's also absolutely socially inept and puts his foot in his mouth at every opportunity.
The locals aren't very relaxing to the mind, either, but at the end of (most) episodes, they come to a kind of grudging understanding.
After awhile, they appreciate their gruff, non-existent bedside manner GP. (Well, most of them anyway.)
He's 'awful' because he's just sooooo clueless! He tries hard (usually) but just ALWAYS gets it wrong because he's so clinical! (For example, when he is kissing his girl after they had stayed up all night watching over a patient, after they part from the kiss, he tells her she has bad breath and it could be caused by a variety of diseases or neglects, etc. Of course, she takes offense and dumps him. He can't understand why because he feels that saying the facts would be helping her, etc.)
It's just sooooooooooooooooooooo funny!
